Save time on your next visit to Salas Minor Emergency Center with our web registration facility. Please read below to understand how it works.

• 3. You will be prompted to enter your email address
• 4. You will receive an email with a link to your completed form. Keep this email and use it to return to your completed form on your next visit

Registration:

Web registration will only be processed 30 minutes before clinic hours. Please allow 1 hour for a confirmation call from our staff. Make sure to fill out this form completely and the nurse will call you when your room is ready. You must be within 15 minutes to use this web registration. Call us for more information.
